You can have full control of your surveys and questions, to add a new Survey just fill out the form and press Save button.
 

Survey Name:		*Survey Description
Sub Option:		*For Sample MS  Office, Part of one sub option
Level:		* LEVEL I, II or III or new one
URL:		* (survey.asp?id=) The url where this survey will go when clicked. This is generated by the system

When you are finished, click on this button to , for processing. Click only once.
If you want to delete all of your answers and start again, click here
